Agreement DOWNLOAD AGREEMENT FORM PLEASE ENSURE YOU COMPLETE AND RETURN BOTH PAGES OF THE FORM Nottinghamshire International Quarantine Centre World Wide Imports and Exports, Quarantine and Boarding Agreement AGREEMENT BETWEEN PET OWNER/AGENT AND NOTTINGHAMSHIRE INTERNATIONAL QUARANTINE CENTRE DEPOSIT AND CHARGES Accommodation will only be confirmed on receipt of the agreed deposit. Deposits and monthly payments may be made by (1) Cheque - payable to Mr R G J Whiteside, or (2) Bank Transfer (details on request) or (3) Cash. We cannot accept credit cards. On receipt of the deposit and faxed Agreement pages 1 and 2, the Import Licence documentation will be applied for. The deposit is non-refundable if cancellation, for any reason, is within six weeks of the agreed import date. We accept fair wear and tear to equipment within your pet's accommodation, however, if the damage is deemed unreasonable an agreed charge will be applied. TRAVEL INFORMATION We do request owners to advise us of their pet's route i.e. which airline, airport of departure, airport of arrival etc. We are licensed carrying agents and our collection, carrying and handling charges included up to two hours waiting time at an airport or port of entry. HM Customs may levy VAT on the value of the animal and the cost of the freight under certain circumstances. A form C5 will be sent or faxed to any client outside the EU for completion and return to avoid VAT being paid on their pet. At some UK airports (depending on the Airline), the Airport Quarantine Station's holding fees may also be incurred. All airline/airport cleaning and handling charges are payable by the pet owner and these costs are added to the first invoice which is payable when the pet arrives. ACCOUNTS Monthly accounts are sent in advance and must be paid within 7 days of the due date. Interest at the rate of 10% will be added to the invoice after a 7-day period following the due date. Any cheque's returned by the bank "refer to drawer" will carry a £35 credit charge and only cash payments thereafter will be accepted. If any account is outstanding at the end of the quarantine period, the Quarantine Centre reserves the right to transfer the pet to the boarding section and hold the animal until all outstanding charges (including boarding fees) have been paid in full. If the animal is left following notice of intent thereof, it will then be re-homed. PET POSSESSIONS We are happy for owners to enhance their pet's accommodation but owners must understand that if any toys cannot be cleaned and are damaged at the end of the quarantine period they will not be returned to he owner when their pet is released. Any bedding brought in will be destroyed. The Centre shall not be responsible for loss or damage to any such items. VETERINARY FEES The DEFRA licensed Veterinary Surgeon for the Quarantine Centre is Raoul S Dowding, 8 Morton Terrace, Gainsborough, Lincs DN21 2RF. Tel 01427 612188. The VSO (vet) visits each pet six days a week for fitness and health. A charge per month is made for these visits. Andy medication given is extra and a small surcharge may be levied if nursing or supervision is required. We reserve the right to call out the Veterinary Surgeon at any time at the owner's expense if we feel it necessary or urgent. We will endeavour to contact the owner prior to any emergency. Private insurance to cover veterinary fees can be arrange privately with the proprietor. Should your pet not survive the quarantine term for whatever reason, the quarantine fee would cease from that date. All pets can be regularly groomed, bathed, clipped, scissored or manicured by a Professional Groomer at the request of the owners, all fees are to be met by the owner. Owners are encouraged to groom their pets when they visit. VISITING We are open for visiting strictly from Wednesday to Saturday 1400 - 1600 each week. We are CLOSED Bank Holidays and all other times. We ask for all visitors to adhere to the Ministry Rules and Regulations at all times whilst on the premises and children must be under strict control of an adult. Prior arrangements may be made for a Sunday visit only if owners cannot visit during the week. TRANSFER OR RE-EXPORTATION Should you request your pet to be transferred from NIQC to another Quarantine kennels during the agreed quarantine period, we would require ONE months notice and or the due monthly fee paid in lieu. We can apply for and comply with the PETS scheme for any owner wishing to take their pet abroad following the six month quarantine period. PET PASSPORT SCHEME Any pet that partially completes the requirements of the Pet Passport Scheme can be quarantined and application will be made for early release. A minimum of ONE month's quarantine will be charged as well as collection, customs/airline handling charges, AQS, veterinary fees, administration and license to release
